our practice to your insurance company. It is imperative that we have the
correct insurance information for you during each of your visits, so that we may

We will
help you in any way we can to work with your insurance company to achieve
maximum benefits; however, it is ultimately
your responsibility to know your health insurance policies and the benefits
covered under your plan. While our billing staff can submit your visit to
your insurance company (under most circumstances you will not need to submit
your own claim form), only your insurance company can determine which services
are covered benefits under your plan.
Our practice accepts most insurance plans; the most accurate way to insure
that we accept your insurance company is to call your insurance company (the
phone number is usually listed on your insurance card).
