Beginning your care with our practice is an important step in your plan for wellness. It’s your opportunity to openly discuss your health with a skilled practitioner who can provide you with education, guidance, and information.
For certain types of visits, we may ask you to complete and return forms to our office which may include questions about your personal and family history. If applicable, please be sure to return the forms to us prior to your appointment to make your visit most efficient.

It is best to call your insurance company to verify your benefits prior to being seen.
Request a referral from your primary care practitioner’s office if your insurance company requires a referral. This may depend on the type of services you receive. If you are unsure if a referral is needed, contact your insurance company to find out.

We are here to make your first visit with our practice as comfortable and as efficient as possible – our friendly and competent staff are here to help. Knowing what to expect during your first visit is important in getting to know our practice.
What to bring
Our receptionist will check you in for your appointment, confirm your demographic information, and collect your copayment if applicable. The receptionist may ask you to complete paperwork related to your visit.
Your next stop will likely be in our lab area within our suite. Depending on the type of visit you are having, the Medical Assistant may ask for a urine sample; we may also take your blood pressure, height, and weight and ask questions related to your health and your family’s health. If you are uncomfortable answering any questions, talk to your practitioner about your concerns – we’ll be happy to explain a question or why the information is important. Being open and honest will help your practitioner address your health appropriately.
Depending on the type of visit, the Medical Assistant may ask you to change out of your clothes and undergarments privately into a smock (or “johnny”). Your practitioner will be in to see you shortly after that. Before your practitioner starts your exam, they will talk with you about your health and address any concerns or questions you may have. You will have the opportunity to ask questions throughout your visit, but it is best to let your practitioner know what your concerns are at the beginning of your appointment so they can address your questions appropriately to meet your needs and expectations.
Depending on the type of appointment, your visit may include a physical exam with components such as a breast exam and pelvic exam. The components of your exam will depend on your individual circumstances and health needs. At any time you can ask questions.
The practitioner will conclude the visit and answer any remaining questions you may have. Your next stop will be at the front desk area where our receptionist will complete the check out process including scheduling any follow up visit(s) needed.
