Your first visit to our practice
Things to do before your first visit
- Request your previous medical records, if applicable.
- If your first appointment with us pertains to an established problem for which you have received medical care elsewhere, please arrange for transfer of these records so we have them in hand on the day of your appointment. This will allow your practitioner to provide continuity of care and will reduce unnecessary duplicity in diagnostic measures or treatments.
- If your first appointment is for a routine exam or a new problem, we do not require your previous medical records. However, you are welcomed to transfer your records if they contain pertinent gynecologic medical history.
- Please visit our records resource page for further details on transferring your records to our practice.
- Give yourself enough time to return forms noting “return by” dates. Some of these forms may require pertinent family history. For your convenience, we have listed the most common types of paperwork by appointment type for new patients. If you are unsure if you need to complete paperwork, please contact our office.
- Verify your insurance benefits and request a referral, if applicable
- It is best to call your insurance company to verify your benefits prior to being seen.
- Request a referral from your primary care practitioner’s office if your insurance company requires a referral. This may depend on the type of services you receive. If you are unsure if a referral is needed, contact your insurance company to find out.
